Living with a health problem is hard, but support changes everything. The Ascension St. John Bedrock Initiative provides the help you need to stay on track.
Program Overview
Some days, taking care of your health feels confusing or overwhelming. You may not know what to do next. We are here to help. The Bedrock Initiative at Ascension St. John gives you someone you can count on.
Community Health Workers
Community health workers are invested in the public's health and trusted members of the community they serve. They act as intermediaries between the individual and health systems to advocate, engage and support people along their healthcare journey.
Ambassadors
A Bedrock Ambassador is a trained volunteer champion selected to represent and promote the Ascension St. John Bedrock Initiative within their faith-based or community-based organization. They advocate for the recognition and integration of the Initiative's goals through specific health promotion activities.
Wellness Support Services
Enrolled members can access these services:
- Wellness screenings, cardiovascular scans*, and blood pressure checks
- Biometric and nutritional assessments*
- Physical health and education programs
- Connection to free or reduced-cost services like healthcare, housing, transportation, job training and more
*Participation is subject to eligibility requirements.
